Please complete the forms attached below by printing them and bringing them with you to your initial evaluation.
We kindly ask that you arrive 20 minutes before your initial evaluation so that we can collect your forms, photo ID, insurance card (see accepted insurance plans here), referral from your physician (if applicable) and your medication list prior to you meeting with your Physical Therapist. Remember to wear comfortable clothing and tennis/supportive shoes.
